A Few Things You'll Need

After you have familiarized yourself with the Sample Practice, you are ready to begin setting up your own practice. It is recommended that you gather the following items together before you begin.

List of Diagnosis Codes (ICD codes) commonly used in your practice.
List of Service Codes (CPT, HCPCS, custom codes) commonly used in your practice.
List of popular insurance carriers used in the practice. (Names, addresses, telephone numbers, EDI payer identification numbers etc.)
List of active patient files. “Active,” is defined as patients you are currently treating.
Provider Insurance Information. (Insurance identifier numbers, group identifier numbers, individual and group NPI numbers).
